Announcement

The Seminar/Workshop on the Manual of Operations for Analysts of Drug Testing Laboratories which will be conducted by the National Reference Laboratory - East Avenue Medical Center (NRL-EAMC) is scheduled on November 26-28, 2008 at the East Avenue Medical Center. The seminar/workshop is from 8:00AM - 5:00PM.

If the participant is outside Metro Manila, please download the pre-registration form at www.doh.gov.ph/nrl ( click the announcement button). Send the filled up pre-registration form together with your payment (postal money order)--payable to East Avenue Medical Center thru LBC or JRS to:

Terms and conditions payment:

1. The Seminar/Workshop fee is P3,500.00, inclusive of training kits, two (2) snacks and lunch. A pre-registration fee of P1,000.00 is required. These shall cover for the reservation of food and related supplies during the seminar.

2. The participant may pay the full amount of the seminar/workshop fee. However, in the event that he/she fails to attend on the scheduled training date without prior notice, the pre-registration fee of P1,000.00 is forfeited from the P3,500.00

3. In case there is a change in the name of participant, the NRL-EAMC should be notified in writing before the start of the scheduled date.

4. In case of cancellation, please notify NRL-EAMC through mail, fax, and email or telephone one week prior to the actual training schedule. However, if the notice of cancellation is less than a week prior to actual training schedule th pre-registration fee of P1,000.00 will be forfeited.
